- You used your personal credit card or debit card at a Sprouts grocery store in the United States between 16 August 2020 and 31 October 2022 (credit and debit cards, excluding EBT cards), or between 15 March 2021 and 15 April 2023 (EBT cards).
- You were given an electronically printed receipt at the point of sale on which more than the last five digits of your card number was printed.

- If you received written notice by e-mail with a Notice Number beginning with the letter P: a Short-Form Claim Form attesting that at least one transaction in the records was made with your personal credit or debit card.
- If you did NOT receive written notice by e-mail with a Notice Number: Claim Form-R, together with the required supporting documentation.
- Your completed claim form must be submitted online or mailed postmarked no later than 5 August 2026.
